WHIPPET RESCUE

ADOPTION AGREEMENT
Please read the conditions listed below for adopting this Whippet and if you agree, sign where indicated.
  1. The Whippet you have adopted will be given 3 to 4 weeks to adjust to new surroundings in consideration of the fact that it will take this long for a Whippet to become compatible with a new environment. During this trial period, the adopter is legally responsible for said animal. NCWFA, Inc. Whippet Rescue cannot be held responsible.
  2. This adopted Whippet will be returned to Whippet Rescue if under any circumstances the adopter is no longer able to keep said dog. The Whippet can not be sold, bequeathed, or given to anyone other than Whippet Rescue.
  3. The Whippet will wear a buckle collar with a tag, showing the name and address of the adopter, as well as current license and rabies tag at all times. (Adopter will leave tag of Whippet Rescue on until new tag with adopter's name and address is obtained.)
  4. If the Whippet is accidentally lost or stolen, the adopter will immediately notify Whippet Rescue. The adopter also agrees to vigorously pursue the recovery of the Whippet, including notice in at least one major local newspaper, plus area posters.
  5. The Whippet will be licensed by the adopter with the jurisdiction in which the adopter resides within 4 weeks from date of this adoption agreement.
  6. The Whippet will be kept on a leash whenever taken out of the adopter's yard for protection against running away or being hit by a car. Local leash and dog laws will be observed.
  7. The Whippet will be given the necessary rabies, distemper, hepatitis, leptospirosis, corona virus, and parvo virus boosters on a yearly basis. This Whippet was vaccinated on:______________________
  8. The Whippet will be taken to a veterinarian if sick or injured. If the adopter is unable to do so, Whippet Rescue will be notified immediately.
  9. The Whippet will live indoors. Food and water will be provided for the Whippet. Off-lead exercise will be in a fenced area. The Whippet will not be tied or chained up.
  10. The adopter will, if applicable, obtain his/her landlord's permission to have a Whippet prior to this adoption.
  11. The adopter agrees to notify NCWFA Whippet Rescue of any changes in address or telephone number.
  12. I understand that the NCWFA Rescue person adopting out the Whippet or his/her representative may visit my home without giving advance notice. If at any time the NCWFA Rescue determines that the terms and conditions of this agreement have not been met or that the animal is not receiving proper and humane care, the NCWFA Rescue person may take immediate custody of the animal.
  13. The adopter acknowledges that the Whippet, which is the subject of this agreement, has been taken in by NCWFA Whippet Rescue and that Whippet Rescue has little or no knowledge concerning the background or previous environment of this Whippet. Whippet Rescue in no way warrants or guarantees this Whippet. The adopter hereby releases, discharges, and holds harmless the representative, Whippet Rescue, and any other person, firm, corporation, or association charged with or chargeable with liability including their heirs, administrators, expenses, loss of service, actions, and causes of action which the adopter now has or may hereafter accrue on account of or in any way growing out of any and all known and unknown, foreseen and unforeseen injuries, property damages, and the consequences thereof resulting from the placement of this Whippet with the adopter.
  14. The adopter agrees to comply with all conditions of this agreement. Failure to comply with these conditions will be sufficient cause for repossession by Whippet Rescue.
  15. Whippet Rescue agrees to accept the return of this Whippet at any time for any reason.
